for CWD Foundation (SCWDF) is a branch of the Children with Diabetes
Foundation, a non-profit 501(c)(3) organization.
The mission of SCWDF
is to provide short-term diabetes supplies for children with type
1 diabetes who are in emergency situations in which their families
are unable to obtain the basic supplies for diabetes care. An emergency
situation might be defined as loss of health insurance, loss of
a parent’s job, or a local disaster, combined with the family
having no other resources with which to purchase diabetes supplies.
Diabetes supplies is defined as any of the following: blood glucose
meter, blood glucose test strips, insulin, insulin pump supplies,
blood or urine ketone strips, lancets, syringes, and glucose tablets.
Short-term provision of supplies is defined as up to three months.
Supplies will be provided one month at a time, as funds allow.
Families requesting
assistance will need to complete a brief application describing
what supplies are needed and the situation that caused this need.
There does need to be evidence of an emergency situation and no
additional backup resources for the family in order to be considered
for assistance. We are trying to specifically target those children
who would not otherwise have supplies. An adult SCWDF volunteer
will verify the situation with the family by phone or by email (if
you have provided an email address, please do check your mail).
Supplies will be purchased
with SCWDF funds through a participating pharmacy (defined as one
which will offer diabetes supplies at a reduced price for the purposes
of this Foundation). The pharmacy will send the supplies directly
to the family. Original prescriptions are necessary for prescription
items (or the physician may fax a prescription to the participating
pharmacy).

. Checks
should be made out to CWD Foundation, with a notation stating ‘supplies’
in the memo line. They will be processed and a thank you letter
will be sent within one week, acknowledging the receipt of the tax-deductible
donation. The money will then be sent to the CWD Foundation main
office. Money that is donated to SCWDF will only be used for the
purchase of supplies and shipping.
